Athlete Injury/Accident Report

PLAYER INJURY PROTOCOL

Coaches: Should one of your players sustain an injury, please follow the below protocol and complete the Injury Report above.

Coach Preparedness & Awareness

  1. Exercise caution & conservatism with all injuries
  2. Be vigilant about player safety
  3. Ensure First Aid kits are complete, intact and present for all practices and games
  4. Ensure access to a telephone if necessary in the event of an emergency
  5. Possess parents/guardian telephone numbers for all practices/games
  6. Contact parents/guardian if injured or symptoms of injury
  7. “When in doubt, sit them out” This means any player showing any signs of concussions will be sat out immediately. 

Please seek medical attention if an injury results in a player temporarily discontinuing their participation. A player shall not participate in any practice, training, or games without release by a licensed Medical Professional for severe injuries including concussions. Injured players are encouraged to continue to attend team events during their recovery if cleared by their physician.

How to recognize a concussion?

 

The following injuries constitute a medical emergency and require immediate medical attention:

• Blockage or stoppage of airway, breathing or circulation

• Loss of consciousness

• Any type of seizure

• Severe bleeding

• Severe fracture, dislocation or deformity

• Any injury to the head, neck or spine

• Heat illness: Change in facial color or appearance, extreme fatigue, disorientation, or loss of consciousness.

• Diabetic Emergencies

• Severe asthma or allergy attack

In the event of a medical emergency the following steps should be taken:

  1. If an ambulance is needed, call 911. 
  2. Give the dispatcher as much information as possible as it pertains to the emergency and your location. 
  3. Contact the parent/guardian of the athlete only after emergency medical response is activated.
